American Indian Health Service of Chicago accepts most insurance plans. In order for us to better serve you; please call the clinic before your visit to make sure we accept your insurance plan. American Indian Health Service of Chicago also offers self pay sliding scale, which adjusts according to your family size and income. However, any co-pays, deductibles, or non-covered services must be paid at time of services rendered. We also accept Cash, iPay, and Credit Cards.

  • Lay-off notice from last employer
  • Current information from Unemployment Office
  • Denied Medicaid application and reason for denial
  • Check stubs from Unemployment (last 4, if possible)
