American Indian Health Service of Chicago will be offering No Cost or Low Cost physicals and immunizations for the entire month of August. Appointments are available Monday-Friday from 9:00 AM- 3:30 PM. *FREE for qualifying families with tribal documentation. LOW COST for all families. Based on availability of services.
